If The USA Gets The Absolutely Necessary Medicare For All, It Will Start In A Single State

But not from California, Vermont, or Connecticut.  Those assholes chickened out or were dumb enough to elect liars who had no intention of instituting a medicare for all type program.  But if it happens, it will be like Canada, staring in Alberta and moving nationally a stop at a time.
